PEO Health Insurance

Health insurance is the benefit that sells most PEO contracts. Under co-employment, worksite employees can access health, dental, vision, life and retirement benefits through plans the PEO arranges, which lets a ten-person firm offer cover shaped like a large employer's. The mechanism is real, but it comes with structure worth understanding: who sponsors the plan, how the arrangement is regulated, and what happens to your people's cover on the day you and the PEO part ways.

How a PEO delivers health benefits

NAPEO, the industry association, lists health insurance among the standard benefits PEOs provide alongside dental, vision, life cover and retirement plans. The commercial logic is aggregation: the PEO negotiates with carriers across its whole client base, so small employers access plan designs, networks and administration that would be unavailable or unaffordable alone. Structurally, either the PEO sponsors a plan covering worksite employees across many clients, or it administers arrangements where each client remains the sponsor of its own plan. The two look identical on an enrollment screen and behave differently in law, so the plan documents, not the brochure, tell you which one your employees are in.

The questions that matter before enrollment

Ask who the plan sponsor and named fiduciary are, and request the summary plan description rather than the marketing summary. Ask how premiums appear on the invoice: an itemised premium line lets you compare the PEO's rate against the open market, while a blended per-employee fee hides the markup. Finally, ask what the renewal history has looked like for clients of your size, since a sharp first-year rate followed by steep renewals is a known pattern in bundled benefits.

What happens to cover when you leave the PEO

Leaving a PEO usually means leaving its health plan, because the plan belongs to the arrangement, not to your company. Your employees' continuation rights, replacement cover and the timing of the transition need answers before you sign, not at exit: what notice the contract requires, whether cover runs to the end of a paid month, what COBRA or state continuation applies, and what data you receive to stand up a successor plan.
